How to send questionnaires for clients to complete:
Please click on the questionnaire you wish to have your client complete.
Once on the questionnaire page, copy its url
Send url via email to your client/s
Once clients click ‘submit,’ they are redirected to the Thank You page and we receive an email with their answers in pdf format. That file can be saved to their Jane App client profile.
Questionnaires are powered by Hushmail for Healthcare platform, which is encrypted and PHIPA-compliant.
