We are thrilled to work with you in providing your patients/clients with the best care possible.
By providing your patient's contact information* below, you confirm that the patient has consented to the referral on their behalf. In addition to the referral form below, if possible, please send a copy of the referral form to hello@prismapractice.com.
* Please ensure that your patient knows and consents to the referral.
* We will contact the patient twice by phone and email and will inform you if we cannot reach them.
All messages are received by PRISMA staff only, but email messages are not always secure. Your continued use of email communication confirms that you accept this risk.
If your patient is in need of immediate help, please ask them to call 911 or go to the nearest hospital.

PRISMA is centrally located in the heart of Yorkville Village in downtown Toronto.
Our building is wheelchair accessible and offers all-gender washrooms.
